Betamethasone Dipropionate is a high potency corticosteroid that is used as an active ingredient in several medications to treat dermatitis, psoriasis, and eczema. This chemical is a synthetic analog of adrenal corticosteroids that are produced in the body.
Betamethasone dipropionate was first patented in 1987 and after the patents expired (in 2003 and 2007 respectively), it is now considered a generic drug.
It is a 17,21-dipropionate ester of betamethasone that has anti-inflammatory and immunosuppressive actions and is in the form of white crystalline powder.
IUPAC name: [2-[(8S,9R,10S,11S,13S,14S,16S,17R)-9-fluoro-11-hydroxy-10,13,16-trimethyl-3-oxo-17-propanoyloxy-6,7,8,11,12,14,15,16-octahydrocyclopenta[a]phenanthren-17-yl]-2-oxoethyl] propanoate

Clotrimazole and betamethasone dipropionate cream is a topical medication that is used to treat fungal infections like ringworm, athlete’s foot, and jock’s itch. Due to its anti-inflammatory nature, it can also be used to reduce and help inflamed skin characterized by redness, itching, and swelling.
Betamethasone valerate is also a synthetic corticosteroid that is used in the treatment of skin issues in form of gel, ointments, creams, solutions, and lotions. Both of these esters are susceptible and sensitive to heat which can cause them to undergo degradation. This can lower their therapeutic activity. Studies show that betamethasone dipropionate degrades slower than betamethasone valerate. There is strong evidence to suggest that betamethasone dipropionate is less susceptible to photodegradation than betamethasone valerate.
The compound can come in multiple forms and specifications. Below are some of them.
This form is an active ingredient that is a glucocorticoid steroid. Drug companies use this ingredient to manufacture and produce drugs that have betamethasone dipropionate.
This is a topical medication in form of a cream with the active ingredient betamethasone dipropionate at 0.05% concentration. It is used to treat skin issues as mentioned above.
This is betamethasone dipropionate in an ointment form. Ointments usually have an oily component in them.
This is betamethasone dipropionate in a lotion form with more water component than oil.
Augmented betamethasone is similar to regular betamethasone in strength but has a higher potency. Augmented betamethasone is known to work faster due to its rapid absorption by the skin.
Betamethasone dipropionate works by activating some specific natural substances in the skin that reduce redness, swelling, and itching. If you look at the specific mechanism of action, it can be seen that betamethasone works by binding to specific receptors. These are intracellular glucocorticoid receptors. Subsequently, betamethasone binds with DNA and alters gene expression. This triggers the synthesis of some specific anti-inflammatory proteins along with inhibiting the production of some other inflammatory mediators that increase inflammation.
The precursor for the formation of betamethasone dipropionate is 1,4,9,16-tetraenepregna-3,20-diketone. During the manufacturing, the precursor is modified at carbon numbers 9,11,16,17, and 21 to obtain betamethasone dipropionate and other derivatives e.g., betamethasone sodium phosphate and betamethasone acetate to name a few. The manufacturing method is modified to use the same precursor as used in earlier methods but the process is simpler than before and it doesn’t require expensive equipment and the final product is manufactured in greater volume than before. The manufacturing process uses an improved form of Grignard reaction where epoxy groups carry out this reaction directly, therefore, bypassing many steps and lowering the cost of production of betamethasone dipropionate.
Betamethasone dipropionate can be used in a variety of ways like:
Can be used to treat dermatitis
Can be used to treat allergies
Can be used to treat rashes
Can be used to treat eczema
Reduces inflammation
It may be used to treat rheumatoid arthritis and systemic lupus erythematosus
Can be used to stimulate fetal lung maturation
Betamethasone dipropionate dosage can differ based on what you are using it for. Generally, it is used as a topical in 0.05% dosage 2 or 3 times a day.
There have been some adverse effects reported after the use of topical betamethasone dipropionate. These can include itching, burning, dryness, hypopigmentation, secondary infection, miliaria, striae, skin atrophy, dryness, irritation, allergic contact dermatitis, maceration of the skin, perioral dermatitis, acneiform eruptions, folliculitis, and hypertrichosis.
